Troubleshooting Prescription/Doctor Note Requests
Purpose
This document has been created to help provide troubleshooting ideas for when an administrator requests a prescription.
Check the Current LMN
In some cases, the letter of medical necessity can include too broad of information or does not clearly show three key points. In this case, start by assessing the LMN to see if it is in need of updates
Keys to Look For
Treatment is specific to a product/service
Dosage is listed
Provider signature is clearly visible on the document
Administrator specific LMN includes Truemed Default Template with it
Treatment Specific to Product/Service
In this example, the letter of medical necessity lists a wide range of products and services instead of just focusing on the product or service that was purchased. This can cause the administrator to be unclear what to focus on and result in a quick denial.
Action Step
Send a request to the provider team to make the letter of medical necessity more specific for the products/services the customer purchased.
Include the name of the product in your request to make it a seamless process for the provider team to update.
Combine administrator LMN template with Truemed Default Template
If the original document only included the administrator specific LMN template, combine this with the Truemed Default Template for a stronger presentation.
Dosage
In this case, the dosage may be a bit too vague for the administrator to approve.
Action Step
Send a request to the provider team to provide a more specific dosage for the product/service purchased.
Provider Signature
The final area to check is that the provider signature is present and also not on a completely separate page of the document. In some cases, a provider signature on its own page can get missed by the administrator.
Action Step
Re-size PDF to have provider signature on the same page as other information.
Utilize the Truemed Default Template if a provider signature is not present and merge with the administrator specific template.
Final Step
When providing the updated letter of medical necessity to the customer, include information on why NPs are allowed to sign the letter of medical necessity. These talking points can be utilized by the customer to help strengthen points where an administrator says a MD is required .

If Doctor Signature Still Requested
If we complete these attempts and a doctor’s signature is still requested, reissue the LMN requesting an MD signature.
